Ohio Child Enrollment Form
The State of Ohio requires every family to complete the Child Enrollment Form (DCY 01234) before a child's first day. It covers parent and emergency contacts, health information, and emergency transportation permission.
- Download the form and fill it in on your computer, or print it.
- Sign and date page 2.
- Bring it to the center or email it to rahmahchildhoodcenter@gmail.com.
Tuition and fees depend on your child's age and schedule. We share current rates by phone or when we reply to your inquiry.
